Summary
Under the direction and supervision of the Program Manager, the Housing Disability Assistance Program (HDAP) Case Manager will be responsible for providing intensive case management services for clients (homeless/chronically homeless) receiving services under the Department of Health Services Housing For Health program. This position will have the overall responsibility for the following job functions:
Essential Functions
Must know, understand, and be able to articulate the mission, vision, and core values of HOPICS Ability to understand and carry out oral and written direction. Provide a range of supportive services (service planning; housing and rental assistance, housing search and location, linkages to health, mental health, substance use disorder services, and legal services, benefits establishment, transportation, basic needs (food, clothing, hygiene), crisis management, eviction prevention, life skills ect.) transitioning from homelessness to permanent housing.
Minimum Qualifications - Knowledge, Skills and Abilities Required
• Bachelor's degree, 2 years in Social Service Field, fully vaccinated
Supervisory Responsibilities:
• No supervisory responsibilities for this position
Environmental Conditions (Working Conditions)
• Work in the office and field, Problem solve, work w/aggressive clients, work in various shelter.
Physical Requirements
• In the course of performing this job, the incumbent typically spends time sitting, standing, walking, speaking, thinking, listening, and carrying (20lbs). Must sit at computer for data entry and reports: sometimes spend time driving and walking and carrying equipment/files/charts. Assisting client with moving their personal belongings. Speak to staff and outside agencies in public meetings and Provider Meetings. Represent the Agency in a professional and positive manner.
