Summary
The SSG HOPICS Homeless Treatment Program (HTP) provides comprehensive support to single adults (18+) in Service Planning Area (SPA) 6 who are experiencing homelessness, trauma, and substance use challenges. HTP offers a trauma-informed, client-centered approach that includes individual therapy, case management, substance use counseling, and gender-specific group therapy using the Trauma Recovery and Empowerment Model (TREM).
The HTP team will play a key role in providing therapeutic support, crisis intervention, and case management to clients, helping them navigate their recovery journey and access essential services such as housing, employment, and healthcare. Services are delivered in various settings, including in-office, field-based, shelters, motels, and via telehealth, ensuring accessibility and engagement.

Essential Functions
  • Mission & Program Knowledge: Understand, articulate, and align with HOPICS’ mission, vision, and core values, maintaining knowledge of all programs and eligibility criteria.
  • Client Engagement & Assessment: Screen referrals for eligibility, coordinate intakes, complete non-clinical intake documents, and administer screening tools. Develop and update individual housing and service plans (HSPs) every three months or sooner, based on client progress.
  • Housing & Supportive Services: Assist clients in securing permanent housing by coordinating housing placement services, rental assistance, and community reintegration support. Provide post-discharge resources for housing stability and homelessness prevention.
  • Case Management: Maintain a caseload of 20-45 clients, conducting 90% of services in the field and 10% in-office. Provide intensive case management, addressing barriers to stability, employment, and self-sufficiency.
  • Client Support & Linkages: Facilitate access to mental health, substance use, healthcare, legal aid, benefits, transportation, crisis management, and life skills development to support long-term stability.
  • Service Coordination: Work collaboratively with internal teams and community providers to coordinate client services, treatment referrals, and interagency support.
  • Group Facilitation & Skill-Building: Conduct life skills groups focused on practical skills such as budgeting, time management, and communication. Facilitate Trauma Recovery and Empowerment Model (TREM) groups, providing structured, gender-specific, evidence-based therapy to help clients process trauma, build coping strategies, and support their recovery journey.
  • Documentation & Compliance: Maintain accurate and up-to-date client records in accordance with HIPAA, CFR 42, and funding requirements, entering data into the EHR system within 24-48 hours. Case notes must follow the G.I.R.P. (Goal, Intervention, Response, Plan) format to ensure consistency and clarity in service documentation.
  • Outreach & Advocacy: Conduct outreach to potential recipients, facilitate internal referrals, and coordinate with approved treatment providers as needed. Represent HOPICS professionally at meetings and community events.
  • Training & Meetings: Attend departmental, funding-specific, and division-wide meetings and training as required. Stay informed on program updates and best practices.
  • Client Transportation: Coordinate and provide transportation for participants using Agency vans when needed.
  • Professionalism & Boundaries: Maintain appropriate professional boundaries with staff, clients, and partners, demonstrating ethical practice and integrity in all interactions.
  • Other Duties: Perform additional responsibilities as assigned by the Program Supervisor, Senior Manager for Mental Health, and/or Associate Director.

Minimum Qualifications - Knowledge, Skills and Abilities Required
  • Bachelor's degree from an accredited university OR two years’ experience working in the social service field focused on mental health, addiction treatment, case management, or counseling.
  • If in recovery, a minimum of three years of sobriety from drugs and alcohol is required.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  • Strong verbal and written communication skills.
  • Ability to work effectively under pressure while maintaining quality standards.
  • Experience working in interdisciplinary teams (homeless services, substance use, mental health).
  • Ability to recognize and report legal issues, including Tarasoff warnings, child abuse, vulnerable adult abuse, danger to self/others, and grave disability in accordance with the law.
  • Verification of employment eligibility and background clearance.
  • Ability to understand and follow oral and written instructions.
  • Proficiency in Microsoft Word, Excel, and internet-based applications.
  • Experience using Excel, HMIS, EXYM and other database programs is preferred.
  • Familiarity with community-based treatment providers is a plus.
  • Ability to outreach and engage clients in diverse settings, including community settings, office spaces, homes, shelters, and other field locations.
  • Ability to coordinate and submit client documentation in a timely manner.
  • Valid driver’s license and reliable transportation with current proof of auto insurance.
  • Willingness to perform duties in various environments and work some weekday evenings and weekends as needed.
  • CPR and First Aid certifications must be obtained within 30 days of employment.
  • TB test required (within three months prior to or seven days after hire, with annual renewal).
  • Participation in community events and engagement efforts.
  • Professional demeanor with a neat professional appearance.
  • Strong attention to detail and punctuality.
  • Bilingual skills preferred but not required.

Supervisory Responsibilities:
  • This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
  • This position requires working in dynamic and sometimes unpredictable environments, with potential exposure to aggressive or hostile clients, including individuals experiencing mental health crises or behavioral challenges. The nature of the work may trigger personal trauma, making it essential for staff to practice self-care and collaborate with a supportive team. Regular interaction with multiple collaborative partners is required, including Los Angeles County Law Enforcement Agencies, Department of Mental Health, Victim Advocate Representatives, the Department of Children and Family Services (DCFS), and other community organizations.
  • Local travel is necessary, requiring reliable transportation for attending court hearings, client visits, and community meetings. Work settings may include noisy environments where clients express themselves freely, often leading to loud or emotionally intense discussions.
  • Additionally, while traveling to training or performing job duties, staff may be exposed to environmental factors such as varying weather conditions, noise, moving vehicles, high-crime areas, odors, fumes, pollen, and other external elements. This role demands adaptability, resilience, and a commitment to working in an engaging, fast-paced, and client-centered environment.
Physical Requirements
  • Light physical effort which may include occasional light lifting (25lbs), and some reaching, bending, stooping, walking, squatting, listening, and typing. A great portion of the day requires sitting, standing, carrying equipment/charts/documents, walking, reading, writing, and communicating or a combination of the above.
Mental Requirements
  • The case manager must be able to perform effectively in unpredictable situations, including crises, constant distractions, interruptions, and uncontrollable changes in priorities and work schedules. This role requires the ability to process complex information, think critically, and conceptualize solutions in a fast-paced environment. Additionally, the case manager must be capable of recognizing and managing transference and countertransference while maintaining professional boundaries and emotional resilience.
