Summary
Under the supervision of the CENS Program Manager and CENS Navigator Supervisor the CENS
Navigator (Bilingual) will complete brief American Society of Addiction Medicine (ASAM) screenings
in English and other languages. CENS Navigators (Bilingual) are responsible for providing information,
linkage/referrals, coordination, and transportation to CENS clients as needed. CENS Navigators
(Bilingual) are further responsible for ensuring program enrollment and follow-up with clients who are
referred to the CENS by The Los Angeles County Substance Abuse Prevention and Control (SAPC)
contracted co-locations, as well non-referred possible clients (walk-ins) that may need substance used
disorder treatment and/or mental health services. The CENS Navigator (Bilingual) will also assist in
some minor training of the new staff at are assigned SAPC contracted co-locations. CENS Navigators
(Bilingual) will also input data input in the internal tracking systems, document daily interactions with
client and community members in to the PCNX data base. The CENS Navigator (Bilingual) position
involves travel between sites daily and is part of the core duties of the position.
Under the direction of CENS Management, the Incumbent will be responsible for:
Essential Functions
Administer the brief ASAM screening in PCNX with clients who are referred to the CENS by
participating agencies, who may need substance abuse treatment and/or mental health services.
Assist in creating client profiles, notes, assessments, and linkages in PCNX.
Complete CENS intake packets.
Will provide clients with information and referrals for treatment services and other ancillary
services.
Coordinate with SAPC approved treatment providers as identified in Service and Bed Availability
Tool (SBAT) for Service Planning Area 6 and client’s needs.
Provide referral and linkage services to appropriate treatment facilities as determined by the ASAM
Brief Screening level of care as well as client and case managers interview.
Provide Navigation Services to ensure successful enrollment to a treatment setting as warranted.
Facilitate At-Risk workshops for CENS clients and staff at the assigned CENS sites.
Facilitate SUD education and workshops for individuals and groups, recovery support, and harm
reduction services.
Coordinate arrangement and transport clients to treatment locations
Link clients to essential services, housing, mental health, legal services, and additional need
Assist client in enrolling and verifying insurance (Medi-Cal)
Assist with the coverage of all CENS co-locations and programs as needed.
Participate in the SAPC monthly and quarterly meeting to meet funding stream specific
requirements.
Provide update and report during the SAPC Quarterly Meetings.
Participate in the SPA 6 Quarterly Providers Meeting
Communicate with co-location and community partners to coordinate services.
Develop and submit weekly client charts to the program manager, supervisor, and/or program
coordinate.
Guide client through the SUD system of care in-network and out of-network
Responsible for preparing project reports in accordance with funding requirements.
Conduct follow-up with clients screened and enrolled to SUD treatment programs.
Provide CENS management with all required client information and screening outcomes using the
CENS Daily Activity Log.
Communicate with Providers and community partners.
Perform duties based on professional practice with State/ Federal Government contracts and grants.
Ensure fidelity to evidence-based practices.
Deliver services using the 12 Core Functions of Substance Abuse Counseling
Submit daily, weekly, and monthly reports in a timely manner.
Maintain appropriate boundaries; and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
Represent the Agency in a professional manner at meetings and community events.
Adhere to SSG/HOPICS Dress Code.
Regular attendance required.
Other duties as needed.
Secondary Functions
Attend all meetings associated with the CENS and other projects as assigned by CENS Navigator
Supervisor, Program Manager or Division Director.
Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other
funding requirements for auditing purposes.
Perform other duties as assigned by the CENS Navigator Supervisor, Program Manager or Division
Director
Other duties as needed’ and ‘overtime, holiday, or weekend work may be required.
Minimum Qualifications – Knowledge, Skills and Abilities Required
Certified or Registered Alcohol and Drug Abuse Counselor. Registered employees must complete
the certification process within five (5) years of their initial registration date.
Bilingual- Spanish preferred
Verification of Employment Eligibility and Background Check
Must be able to visit multiple same day locations within 30 minutes of one another
If in recovery, a minimum of three (3) years of being drug and alcohol free is mandatory.
Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
Ability to communicate effectively, both written and orally
Knowledge of core competencies of Substance Abuse Counseling
Working knowledge of Microsoft Word, Excel, PCNX, and other database programs
Knowledge of various community-based treatment providers
Experience working with the criminal justice population.
TB Clearance upon hire and thereafter annually
CPR and First Aid upon hire and thereafter
Maintaining a smart phone for authenticator use; company cell phone or stipend
reimbursement for personal cell phone use.
Valid California Driver License, Proof of Car Insurance, Reliable transportation
Supervisory Responsibilities
This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
The environment for this position is office and field based.
Must be able to work in an environment with many priorities, busy, and fast paced.
Must be able to solve problems and adapt to changes that are unpredictable.
Must be able to work with aggressive clients.
Exposed to aggressive clients that at times many be noisy, under the influence of an unknown
substance, loud, and disruptive.
Physical Requirements
While performing this job, the incumbent typically spends time sitting, standing, walking, speaking,
thinking, listening, and carrying (20lbs). He/ She will spend time each week providing oversight to Co-
Location sites He/ She must sit at computer for data entry and reports: sometimes spend time driving and
walking and carrying equipment/files/charts.
